Breaking Down Silos, Providing Comprehensive Support

Adults experiencing homelessness face a high risk of health crises and often do not qualify for community support services. This initiative targets those relying on expensive and ineffective solutions for their needs.

NewCourtland has created a housing model that includes social services, allowing individuals to stay as long as necessary. Unlike traditional transitional programs, once someone is housed with us, they receive ongoing support and are considered no longer homeless.

The program has shown a significant reduction in emergency department visits and a high retention rate for participants. Our Housing with Supportive Services program provides integrated health, housing, and social services through a team that includes a nurse navigator, property manager, and social worker.

This “housing-first” approach offers comprehensive healthcare and service coordination, easing the transition and ensuring that individuals do not need to worry about losing their homes. Overall, 83% of residents maintain stable housing and experience fewer health and social crises, all at a lower cost than traditional emergency care.
